血清25(OH)D判断糖尿病合并NAFLD患者继发进展性肝纤维化的应用价值

The application value of serum 25(OH)D in the evaluation of progressive liver fibrosis in diabetic patients with nonalcoholic fatty liver disease

摘要: 目的 研究血清25-羟维生素D[25(OH)D]判断2型糖尿病(T2DM)合并非酒精性脂肪性肝病(NAFLD)患者继发进展性肝纤维化的价值。方法 回顾性分析2015年5月至2019年5月于我院收治的265例T2DM合并NAFLD患者的临床资料,分析其进展性肝纤维化发生情况,并采用多因素logistic回归分析法分析患者继发进展性肝纤维化的独立影响因素,判断血清25(OH)D对进展性肝纤维化的预测价值。结果 265例患者中有80例(30.19%)继发进展性肝纤维化。除年龄、糖尿病病程、体质量指数、血红蛋白、白蛋白、ALT、AST/ALT、血肌酐、胱抑素C、24 h尿微量白蛋白外,血清25(OH)D亦是其继发进展性肝纤维化的独立影响因素(P<0.05),且其对进展性肝纤维化有一定预测价值,曲线下面积为0.831。结论 血清25(OH)D在T2DM合并NAFLD患者继发进展性肝纤维化评估中具有重要作用。

Abstract: Objective To study on the application value of serum 25-hydroxyvitamin D [25(OH)D] in the evaluation of progressive liver fibrosis in patients with type 2 diabetes mellitus (T2DM) and nonalcoholic fatty liver disease (NAFLD). Methods The clinical data of 265 T2DM patients with NAFLD enrolled in our hospital from May 2015 to May 2019 were retrospectively analyzed for their status of liver fibrosis progression. Multi-factor Logistic regression analysis was applied to investigate the independent factors, and to determine the predictive value of serum 25(OH)D for progressive liver fibrosis. Results Of the 265 patients, 80 (30.19%) had progressive liver fibrosis. In addition to age, the course of diabetes mellitus, body mass index, hemoglobin, albumin, alanine aminotransferase, ratio of aspartate aminotransferase and alanine aminotransferase, serum creatinine, cystatin C and 24-hour urinary microalbumin, serum 25(OH)D was also an independent predictive factor associated with liver fibrosis progression (P<0.05) with the area under curve (ROC)of 0.831. Conclusion Serum 25(OH)D plays an important role in the assessment of progressive liver fibrosis in patients with T2DM accompanied with NAFLD.
