幽门螺杆菌感染为胃静脉曲张组织胶注射治疗后排胶出血的可能保护因素

Helicobacter pylori infection as a protective factor for glue extrusion bleeding

摘要: 目的 探究幽门螺杆菌(helicobacter pylori, Hp)感染与排胶出血的关系。方法 收集首都医科大学附属北京地坛医院2008年10月至2017年6月因肝硬化食管胃静脉曲张出血,行胃静脉曲张组织胶注射治疗患者524例,治疗后排胶出血64例,非排胶出血460例,比较两组间的Hp感染率。结果 430例患者检测出Hp阳性,感染率为82.06%,64例患者于治疗后出现排胶出血,排胶出血发生率为12.21%。非排胶出血组患者Hp感染率显著高于排胶出血组(P=0.00)。Hp感染并非排胶出血发生的危险因素(P=0.13),肝脏储备功能差者排胶出血发生率高。结论 Hp感染为胃静脉曲张组织胶注射治疗后患者的可能保护因素。

Abstract: Objective To investigate the relationship between glue extrusion bleeding and helicobacter pylori (Hp) infection.Methods The a retrospective study enrolled 524 patients with endoscopic tissue adhesive injection in our hospital from October 2008 to June 2017.including 64 cases of glue extrusion bleeding group and 460 cases of non-bleeding group. Hp infection rate was compared between the two groups. Results There were 430 Hp positive patients with infection rate of 82.06%, 64 patients suffering glue extrusion bleeding with an incidence rate was of 12.21%. The Hp infection rate in non-bleeding group was significantly higher than that in bleeding group (P=0.00). Hp infection was not a risk factor for glue extrusion bleeding (P=0.130).Bleeding rate was higher in patients with poor liver reserve function.Conclusion Hp infection might be a protective factor for patients with gastric variceal who underwent endoscopic tissue adhesive injection. Additionally,the glue extrusion bleeding incidence has a positive correlation with Child-Pugh score.
