国内肝纤维化临床注册现状

Clinical registration status of hepatic fibrosis in China

摘要: 目的 整理肝纤维化(hepatic fibrosis,HF)的临床研究在中国临床试验注册中心(ChiCTR)数据库中的注册现状,汇总现阶段的研究成果并分析存在的问题,以期为今后临床试验的设计及实施提供借鉴。方法 以“肝纤维化”为检索词,检索ChiCTR 2005年至2024年12月31日与HF相关的所有临床研究。分析HF在ChiCTR中注册的相关信息,包括但不限于注册时间、注册状态、研究类型、方案设计、样本量及是否有经费支持等。结果 共纳入83项HF临床研究,覆盖全国19个省级行政区,临床研究样本量共计29 572例。HF相关临床试验目前注册数量少,研究成果较集中,地域分布不均匀,经费支持占比大,创新型无创诊断试验多样,但其准确性及分期标准有待统一。结论 相关研究者需提升临床试验专业素养,优化试验设计、完善注册信息、规范盲法应用等。应搭建跨区域多机构协同机制并开展高质量临床试验,贴合中医药特色创新研究方案。同时需提升无创HF诊断效能,开展循证研究、探索中医药随机对照试验新模式,促进中医药现代化发展。

Abstract: Objective This paper reviews the registration status of hepatic fibrosis (HF) clinical research in the Chinese clinical trial Registry(ChiCTR), summarizes the current research results and analyzes the existing problems, so as to provide reference for the design and implementation of clinical trials in the future. Methods All HF-related clinical studies in the ChiCTR from 2005 to 2024-12-31 were searched with "hepatic fibrosis" as the search term. The relevant information of HF registration in ChiCTR was analyzed, including but not limited to registration time, registration status, study type, program design, sample size and whether there was financial support. Results A total of 83 clinical studies on hepatic fibrosis were included, covering 19 provincial-level administrative regions across the country, with a total sample size of 29 572 cases. At present, there are some problems in HF-related clinical trials, such as small number of registrations, concentrated research results, uneven geographical distribution, large proportion of financial support, and diverse innovative non-invasive diagnostic tests, but the accuracy and staging criteria need to be unified. Conclusion Relevant researchers should enhance their professionalism in clinical trials, optimize trial design, complete registration information, and standardize the application of blinding methods. It is necessary to establish a cross-regional and multi-institutional collaborative mechanism to conduct high-quality clinical trials and innovate research protocols tailored to the characteristics of traditional Chinese medicine. Meanwhile, it is essential to improve the diagnostic efficacy of non-invasive hepatic fibrosis, carry out evidence-based studies, and explore new randomized controlled trial models for traditional Chinese medicine to boost its modernization.
