肝组织中程序性死亡受体1与自身免疫性肝炎炎性反应的关系

The relationship between programmed death 1 and inflammatory response in autoimmune hepatitis

摘要: 目的 研究肝组织中程序性死亡受体1(PD-1)与自身免疫性肝炎(AIH)炎性反应的关系。方法 选择60例AIH患者为研究对象,根据疾病活动程度分为AIH缓解期组18例、AIH活动期组42例。另选同期60名健康体检者为健康对照组。采用免疫组织化学卵白素-生物素-酶复合物染色法检测研究对象PD-1表达并进行比较,分析PD-1表达与AIH炎性反应的关系,并观察PD-1与总胆红素、丙氨酸氨基转移酶、天冬氨酸氨基转移酶的相关性。结果 AIH患者PD-1阳性表达率显著高于健康对照者(P<0.05),AIH活动期组PD-1阳性表达率显著高于AIH缓解期组及健康对照组(P<0.05)。G0级AIH患者PD-1阳性表达率显著低于G1~G2级及G3~G4级(P<0.05),G1~G2级AIH患者PD-1阳性表达率显著低于G3~G4级(P<0.05)。PD-1表达与总胆红素、天冬氨酸氨基转移酶、丙氨酸氨基转移酶呈正相关(r=0.853、0.863、0.884,均P<0.01)。结论 PD-1与AIH炎性反应活动程度存在一定相关性,提示PD-1可能参与AIH的发生及发展。

Abstract: Objective To study the relationship between programmed death 1 (PD-1) and inflammatory response of autoimmune hepatitis (AIH) in liver tissues.Methods A total of 60 AIH patients in our hospital were selected as hepatitis group, and were divided into remission stage group (18 cases) and activity stage group (42 cases) according to the degree of disease activity. Sixty healthy subjects in the same period were selected as healthy control group. The expression of PD-1 was detected by avidin-biotin-peroxidase complex immunohistochemical staining method, and the relationship of its expression with inflammatory response of AIH was analyzed. The correlations between PD-1 and total bilirubin (TB), alanine aminotransferase (ALT) and aspartate aminotransferase (AST) were studied. Results The expression of PD-1 in hepatitis group was significantly higher than that in healthy control group (P<0.05), besides, the expression of PD-1 in active stage group was significantly higher than that in remission stage and healthy control group (P<0.05). The expression of PD-1 in grade of liver necroinflammation (G) 0 AIH patients was significantly lower than that in G1-G2 and G3-G4 patients (P<0.05). The expression of PD-1 in G1-G2 AIH patients was significantly lower than that in G3-G4 patients (P<0.05). The expression of PD-1 was positively correlated with TB, AST and ALT (r = 0.853, 0.863, 0.884, P=0.000, 0.000, 0.000). Conclusion There is a certain correlation between PD-1 and the degree of AIH inflammation, suggesting that PD-1 may be involved in the development and progression of AIH, which clinical attention should be paid to.
