慢性乙型肝炎血清标志物和肝功能指标在不同性别患者间的差异分析

Analysis of gender disparity of viral markers and liver function indicators in chronic hepatitis B patients

摘要: 目的 探索不同性别慢性乙型肝炎患者病毒学和血清学指标的差异。方法 回顾性研究了155例慢性乙型肝炎初治患者的血清学指标。并在HBeAg是否发生血清学转换的情况下分别比较了不同性别患者:HBeAg或抗-HBe、HBsAg、抗-HBc和HBV DNA:ALT、AST和GGT:TBil、Alb、Glb、PT、INR和PLT的相关性。结果 在HBeAg阴性患者中,男性血清中的HBsAg和HBV DNA显著高于女性患者(分别为1 696 IU/mL比137.9 IU/mL,P=0.029 4;1.84×107拷贝/mL比6.69×107拷贝/mL,P=0.003 9)。HBsAg在男性患者中始终与HBV DNA存在显著正相关(P<0.01),女性患者的这一相关性在HBeAg发生血清学转换后消失(P=0.550 6);在HBeAg未发生转换时,男性患者的HBsAg水平与TBil和PT水平负相关(P=0.0158及P=0.0317)。结论 在慢性乙型肝炎疾病进展的不同时期,不同性别患者不仅HBV DNA和HBsAg水平存在不同,其他血清学指标中也存在显著区别。

Abstract: Objective To clarify the sex disparity of chronic hepatitis B in seum viral markers and liver function parameters.Methods Clinical informations from 155 chronic hepatitis B (CHB) patients were analyzed in this retrospective study. HBeAg or HBeAb, HBsAb, HBcAb and HBV DNA in different genders were compared between the cases with and those without HBeAg seroconversion. The relationship between HBsAg levels and liver injury Indicators including alanine aminotransferase (ALT), aspartate aminotransferase (AST) and glutamyl transpeptidase (GGT), and other serological markers such as total bilirubin (TB), albumin (ALB), globin (GLB), prothrombin time (PT), international normalized ratio (INR) and platelet count (PLT). Results In HBeAg-negative patients, the serum levels of HBsAg and HBV DNA in males were significantly higher than in females. When analysis the correlation of these clinical indicators with HBsAg, it was found that HBsAg positively correlated with HBV DNA in males and in HBeAg positive females. In addition, there are a negative correlation between HBsAg and TB, PT or AST in HBeAg positive males.Conclusion Gender disparity is not only presented in viral markers, but also in other serological indicators in CHB patients.
